We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide essential nursing care to individuals in the comfort of their homes. As an LPN with Advocate Health Care Services, your role will be key in helping clients with their daily living activities, ensuring they receive the care and support needed to maintain their independence and quality of life.
Responsibilities:
- Deliver direct nursing care while adhering to individualized care plans and physicians’ orders.
- Administer medications, injections, and treatments as prescribed.
- Monitor and document vital signs, symptoms, and client responses.
- Assist with personal care tasks such as bathing, dressing, grooming, and toileting as needed.
- Educate clients and their families on health conditions, medications, and care techniques.
- Maintain accurate, timely, and detailed electronic health records (EHR) documentation.
- Communicate effectively with clients, families, physicians, and other healthcare professionals.
- Follow infection control and safety procedures to ensure a safe care environment.
- Respond to emergencies and report any significant changes in condition promptly.
- Participate in care plan updates and interdisciplinary team meetings.
Qualifications:
- Current and unrestricted Licensed Practical Nurse (LPN) license in the state of Minnesota.
- Current and valid Basic Life Support (BLS) certification.
- Minimum of one year of nursing experience, preferably in home care or a related field.
- Proficiency with electronic documentation and medical software.
- Strong organizational and time-management skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and adapt to changing client needs.
- Reliable transportation and a valid Minnesota Driver’s License.
- Ability to lift and assist with physical tasks as required.
- Must pass a background check and drug screening.
Hours:
- Part-time hours with flexible shifts.
- Availability for evenings, weekends, and holidays may be required.
